Planned Absence - If you know of dates that your child will be absent from school, please fill out this form ahead of time and turn it in to the KHS Office.

Medical Appointment Excuse - Students take this form to any medical appointment, have the office at the appointment fill it out, then turn it in to the KHS Office upon return to school.
